A Case of Superior Orbital Fissure Syndrome Attributed to Sinusitis Presenting Unilateral Throbbing Headache

Abstract
We report a case of superior orbital fissure syndrome as a complication of sinusitis presenting with unilateral throbbing. A 74-year-old woman visited our hospital with a 4-days history of severe left side headache. Two days after onset of unilateral throbbing pain, she developed left complete ptosis. An examination revealed ophthalmoplegia without visual disturbance and tenderness in the left maxillary sinus. Brain magnetic resonance image and facial computed tomography revealed dehiscence in posterolateral wall of left maxillary sinus and extension of inflammation to orbit. After 10 days of intravenous antibiotic treatment, headache and ophthalmoplegia were improved. This case illustrates a case of superior orbital fissure syndrome as an unusual complication of sinusitis. Prompt diagnosis and treatment are important to prevent life-threatening complications.
